Manage and extend proactive care across the patient continuum with our FDA 510(k) cleared Care Team Portal, which features cloud-based services and original equipment manufacturers (OEM) biometric accessory devices.

This is a strong solution for patients who have limited technical ability and require a “plug-and-play” option. Services to assist with the deployment and retrieval of the +Home managed health kit are also available.
+Go mobile app
Traditionally aligned with patients that have familiarity with a smartphone, our +Go mobile app is uniquely designed to be a scalable solution that engages large patient populations. Each patient downloads the +Go mobile app to their Android or iOS device and can send biometric data, watch educational videos, complete condition-based pathways, conduct video visits with your care team, and access secure two-way in-app messaging.
This solution is flexible in content and experience and is designed to help engage, empower, educate, and shape patient behavior.
Interactive voice response (IVR)
Patients who do not have access to WIFI or 4G cell service can choose Vivify Voice, allowing the patient or their caregiver to use a landline phone. They are able to answer questions and populate their vital signs. Vivify Voice also supports branching logic.

Additionally, patient utilization is logged and exposed via API (as well as through built-in utilization reports), which integrates with electronic health records (EHR) to provide streamlined clinical experiences.

Clinical implementation and support team
Our implementation methodology focuses on the establishment of a project success plan. We work with your key stakeholders to define program goals and objectives, identify roles and responsibilities, and set clear expectations for time and resources dedicated to meet milestones for each phase of the project.
Operations and logistics
A logistics team at the Optum® U.S.-based, FDA-registered, and Joint Commission Accredited distribution center supports our kits and devices by providing:
- Timely kit delivery to the patient’s home or care facility
- Fulfillment tracking
- Management of the kit receipt and reprocessing after care completion
The team consistently achieves a 99% kit shipment time of two business days.
Common inventory program
Our hardware as a service (HAAS) program will take care of equipment procurement and maintenance — saving your facility the cost of a large capital investment. Devices can be used to serve multiple patients and are consistently updated to ensure the quality of each patient’s experience.
Through this program, Vivify Health provides:
- Monitoring of all OEM device service bulletins for updated information
- Consistent software updates, including security and operating system releases
- Replacement of damaged or underperforming hardware
- Retirement and replacement of equipment at the end of its lifecycle
Education services
Clinical staff can find the information they need, from implementation and beyond, by logging in to Vivify University. Specifically designed for care team members’ individual roles assigned in the Care Team Portal, each module can be digitally accessed and completed on demand. In-person sessions to help ensure all-around adoption and engagement are also available.

Our evidence-based pathways support patients with single and multiple conditions. Clinicians can select from our 46+ pathways to deliver condition-specific questionnaires, biometric prompts, and rich educational content designed to engage patients in the management of their health.
