Bridging the provider-patient gap

Vivify Health’s RPM solutions were created to monitor patients in their homes or wherever they may be. Solutions are based on the levels of observation patients require as well as their specific preferences, including the option to have an at-home kit, a mobile app, or a web-based app. Each monitoring system was made to be simple and intuitive to use¹ and has earned high patient satisfaction ratings. With our solutions, clinicians can monitor data in near real time and intervene before minor symptoms turn into significant issues, aiding to reduce readmission rates and lessen the risk of Affordable Care Act (ACA) penalties.
