Health system will use advanced remote patient monitoring technology to enhance patient compliance with care plans and reduce hospital readmissions.

PLANO, Texas, Feb. 6, 2018 /PRNewswire/ — Yavapai Regional Medical Center (YRMC), the leading healthcare system in Prescott, Arizona, has chosen Vivify Health, the pioneer and market leader of remote care via mobile devices, as its partner in addressing the care needs of patients with advanced congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD). By leveraging Vivify’s platform—including its evidence-based care pathways, instant-on kit of monitoring devices and telemedicine capabilities—YRMC’s care team will be able to continually monitor the biometric data and symptoms of these patients in the comfort of their own homes while aiming to improve their health and quality of life, in addition to lowering their hospital readmission rates.

“We selected Vivify as our partner because we realize that continual engagement with our patients via remote monitoring is key to improving their care and long-term outcomes,” said Boyd Murayama, Director of Physician Practices for Yavapai Regional Medical Center.

Murayama added, “Vivify’s technology will enable our clinical care nurses to monitor these patients every day in the most efficient way possible while providing them with educational materials tailored to their conditions and comorbidities. Additionally, the instant-on health kit provided by Vivify is easy for patients to use without requiring any setup.”

Care nurses can also use Vivify’s dashboard to see whether patients have taken their daily biometric readings and completed the symptom forms on their Vivify-provided tablets. This newfound insight is a significant asset in ensuring patient adherence to care plans.

Indeed, Vivify already boasts a high patient adoption rate. Compared with industry data showing an average compliance rate of 30 to 40 percent among home-monitored patients, Vivify’s partners—including more than 600 hospitals and health plans—have found that over 90 percent of the patients in their remote monitoring programs are compliant with their care plans.

Increased patient compliance and the ability of care teams to continually monitor patients’ health status leads to better outcomes and lowers the cost of care, which is important to organizations that take financial risk. YRMC’s accountable care organization (ACO), for example, has responsibility for a population that includes patients with complex chronic diseases. The health system’s overriding goal is to improve these patients’ health.

Jeff Haggard, Vice President of Customer Development, Western Region, for Vivify, commented, “YRMC’s leadership realized that remote monitoring is a powerful tool for driving better quality of care for patients with complex chronic diseases. Our solution helps patients remain healthy or stable at home and enjoy a higher quality of life. On average, patients in programs supported by Vivify’s platform have fewer acute episodes and readmissions, as well as decreased lengths of stay when they are admitted.”
