PLANO, Texas–(BUSINESS WIRE)–Vivify Health now offers the American Heart Association’s (AHA) Connected Heart Health Initiative, enabling direct patient usage of the AHA’s science-based care plans. This powerful combination of evidence-based Care Plans and technology is the result of a five-year relationship between Vivify Health and American Heart Association.

Connected Heart Health is the American Heart Association’s new initiative to deliver patient-centered care beyond the clinic. AHA Care Plans focus on post-acute/preventative care in several key areas including heart failure, coronary artery disease, cardiac rehabilitation, atrial fibrillation, stroke rehabilitation, hypertension and diabetes.
