PLANO, Texas–(BUSINESS WIRE)–Vivify Health now offers the American Heart Association’s (AHA) Connected Heart Health Initiative, enabling direct patient usage of the AHA’s science-based care plans. This powerful combination of evidence-based Care Plans and technology is the result of a five-year relationship between Vivify Health and American Heart Association.
“Heart disease is progressive, so proactive, preventive measures are most effective at these points in the lifecycle of heart disease.”
Vivify Health provides a Remote Care Management Platform using multiple modalities of interaction for Remote Patient Monitoring, Chronic Care Management, Transitions of Care Services and Population Health. Customers report very high patient engagement and adherence in all age groups, and thoroughly vetted security, privacy and scalability are all important factors in AHA’s determination of platform integration.
Connected Heart Health is the American Heart Association’s new initiative to deliver patient-centered care beyond the clinic. By collaborating with innovators such as Vivify Health, AHA intends to move its trusted information squarely into the lives of today’s healthcare consumers. The initiative is designed to leverage new remote monitoring technologies to deliver better quality of care, fewer re-admissions, reduced Provider costs and, ultimately, healthier lives.
At the center of this initiative are American Heart Association Care Plans. These plans were developed by leaders in preventative cardiology, nutrition and behavioral sciences, and are targeted for use by Accountable Care Organizations (ACOs) and hospitals. They offer Providers with a method to extend evidence-based care, while giving patients personalized tools and guidance to help them adopt better self-management skills and healthier lifestyles. AHA Care Plans focus on post-acute/preventative care in several key areas including heart failure, coronary artery disease, cardiac rehabilitation, atrial fibrillation, stroke rehabilitation, hypertension and diabetes.
Vivify’s platform does contain many standard, customizable care plans for many conditions and is very proud to offer this content expansion of best practice, science-based protocols, complete and ready to implement for heart and stroke patients.
“Together we now empower health care providers and patients with Care Plans from one of the most trusted global sources of evidence-based medicine, with solutions that are scalable in addressing the needs of complex care patient populations,” said Eric Rock, CEO of Vivify Health.
“This is a solid step toward meeting the Connected Heart Health mission 2020 Impact Goal: providing Care Plans designed to promote self-management skills and healthy living, and build health literacy skills for patients following an acute event,” said Patrick Waite, SVP of Patient and Healthcare Innovation of the American Heart Association. “Heart disease is progressive, so proactive, preventive measures are most effective at these points in the lifecycle of heart disease.”
