Reid Health Extends Continual Care to Patients in Their Homes through Vivify Health Pathways

The health system, which serves patients in west central Ohio and eastern Indiana, credits a 90 percent patient compliance rate to attentive onboarding by care coordinators and patient-friendly technology from Vivify Health, the pioneer and market leader of remote care via mobile devices.

With the continual care program launched just this past November, Reid Health is currently collecting benchmark data on reduced hospital readmission rates and increased medication adherence. To that end, each patient in the continual care program receives a remote monitoring kit from Vivify Health that includes a tablet with animated videos that walk patients through an easy set-up, and various wireless health devices to collect the patient’s biometric data.
At any time patients can request a video chat, look up information about their condition, review their care plan instructions, and at scheduled intervals, share biometric data with care team members who monitor for any anomalies in the patient’s condition. Such events can be quickly acted on, often with a mild intervention in the patient’s own home that heads off a more acute deterioration which could end with a hospital admission.
