The +Home service level is generally recommended for patients at a higher risk of worsening conditions and who may have limited technical abilities. Remote patient monitoring with the +Home managed health kit is designed to help improve patient outcomes and enhance both the provider and patient experience.
Our SmartPathTM modules are scalable, standardized, and can be stacked to accommodate patients who may be experiencing multiple health issues. Patients receive their kits preconfigured with their unique pathways. Custom content creation is possible to support patients with conditions not on this list.
