How it works

Providers choose whether to onboard patients at their healthcare facility or ship Bluetooth-enabled, third-party remote patient monitoring devices directly to patients’ homes. These devices, which may include a blood pressure monitor, pulse oximeter, weight scale, glucose meter, pedometer, and thermometer (additional devices available and additional charges may apply), are assigned based on the clinical condition(s) pathway of each patient. Vivify Health provides help with setup.

Accessing +Go

Patients use their mobile phones to access +Go by downloading the Vivify Health app from the app store (available for iOS and Android devices) and following instructions provided by their healthcare providers.

Our SmartPathTM modules are scalable, standardized, and can be stacked to accommodate patients who may be experiencing multiple health issues. Each account is configured with that patient’s specific pathway. Custom content creation is possible to support patients with conditions not on this list.
