Screening. Monitoring. On-Going Care

While screening patients for COVID-19 definitely helps you wrap your arms around the current situation, ‘flattening the curve’ requires a much more holistic approach towards COVID-19 management.

A blueprint for a successful remote COVID-19 management includes a 3-step healthcare delivery model: Ability to Screen, Monitor and provide ongoing level of support that integrates and simulates hospital centered care and support.

Recent reports indicate about 80% of COVID-19 patients show mild to moderate symptoms. The CDC suggests timely triaging and monitoring such patients remotely at home can immensely reduce the strain on healthcare systems and reduce the risk of further spread.

Setting patients in the right direction for a continued path to maintain good health is Vivify Health’s main priority.

As the pressure to act rapidly on Coronavirus continues, enable your organization today to effectively manage your patient population with Vivify Pathways – the industry’s only comprehensive, patient-centered connected care platform.

The Screening

The COVID-19 Screening Pathway enables low-risk patients or those with mild symptoms to use their mobile devices to self-screen for COVID-19 by answering a series of questions that follow the current Centers for Disease Control and Prevention (CDC) guidelines.

Coronavirus Screening Process

The Monitoring

The COVID-19 Self-Isolation and Monitoring pathways enable all patients, using their mobile devices, to continually update their symptoms, monitored remotely by providers.  Additionally, information about other pre-existing health risks such as diabetes, chronic pulmonary obstruction disease (COPD), congestive heart failure (CHF) or any of 90+ other chronic conditions, can also be monitored at home, assuring those at the highest risk levels with COVID-19 receive proper care.

The On-Going Care

Once you have completed screening and monitoring for COVID-19 patients, continue leveraging Vivify Pathways to assess patient’s status through extensive functionality that provides actionable insights to clinical staff for timely care intervention.

How Does RPM Work?

Vivify Health delivers an innovative approach to providing value-based patient care.

We do this by leveraging a virtual platform for provider collaboration that assesses patient status and provides a consistent program for monitoring patient care. Vivify Pathways pulls data from patients wherever they are through their mobile digital devices or at-home remote monitoring kits. This provides actionable insights to clinical staff for timely care interventions. Vivify addresses the health needs of healthy, rising-risk, and high-risk patients, driving stronger adherence rates that improve population health, improve patient satisfaction, and drives additional revenue to your organization.

Monitor & Engage

Vivify offers an intuitive and easy-to-use toolset that continually engages patients, drives positive behavior, and enables timely clinical intervention for effective chronic disease management.

  • Engage your patients when  necessary with a video call instead of a home visit.
  • Fully managed remote care kits drive provider strategies to monitor and engage with high-risk members.
  • When on the go, patients can use their own mobile devices to report monitoring data and connect with care managers.

Continual Stratification

Connected Care blends a patient-centered approach with the best technology available. Population analytics and intelligent algorithms, which can be integrated with EHRs, allow agencies to risk-stratify their populations and decide where to focus their efforts.

Customized Care

Vivify Health begins with over 90 disease-specific clinical protocols that can be easily modified for each patient’s conditions and comorbidities. These include, but are not limited to:

  • Congestive Heart Failure
  • Chronic Obstructive Pulmonary Disease
  • Cancer
  • Hypertension
  • Weight management/obesity
  • Asthma
  • Diabetes
  • Pain management

Data Collection

Vivify Health has the best track record of engagement in healthcare. As a result, our connected care solution is able to collect large stores of data on patients which can be aggregated and analyzed to improve patient panel health, report on quality and power bundled care and quality programs.

Continual Education

Informed patients are more engaged patients. Vivify’s expanding and easy-to-use content library educates and empowers patients while improving health literacy. Content creation and delivery is drag-and-drop easy in our solutions.

Virtual Visits

The value of the video visit to clinical decision making cannot be over-emphasized. The ability to see the patient while assessing their clinical condition via video visit is key to success of the virtual care center.

Patients can use the Vivify solution for one-touch video conferencing with care managers and providers when they want to be seen and on their device of choice.
